Kratom With Liver or Kidney Disease: Safety Questions

If you have liver or kidney disease, kratom is not a question to answer from a general guide alone. Research has not shown how liver or kidney impairment changes kratom’s effects, so the useful next move is to have the person who knows your diagnosis and medicines look at your exact situation.

Why does liver or kidney disease change the question?

Your liver processes nutrients and other substances, while your kidneys filter extra water and wastes from the blood and make urine. The National Institute of Diabetes and Digestive and Kidney Diseases explains that liver disease includes many conditions, and that drugs and alcohol can be among its causes. Its kidney disease overview explains that kidney damage can reduce the kidneys’ ability to filter blood.

That matters because NIDA says kratom’s effects can vary with a person’s underlying medical conditions, other drugs, product formulation, potency, and other factors. Mitragynine and 7-hydroxymitragynine are among the kratom compounds most often studied, but a leaf powder, extract, drink, or product containing other ingredients should not automatically be treated as the same exposure.

If your question began with a label, batch number, or product claim, keep that detail for the conversation. The practical issue is matching the exact product and your health history to the person reviewing your care.

Were people with organ disease excluded from kratom studies?

Current kratom research does not give you a safety answer for liver disease, chronic kidney disease, kidney failure, or other organ impairment. NCCIH says very little research in people has evaluated kratom’s health effects, and NIDA says much remains unknown about its short- and long-term health and safety effects.

That is why healthy-adult findings, laboratory findings, and reports from people without documented organ impairment cannot answer whether your liver or kidney condition changes the picture. They also cannot tell you whether a particular product format fits with your medicines.

What does the liver evidence actually show?

A 2020 review of reported kratom liver-injury cases identified case reports, cases from the Drug-Induced Liver Injury Network, FDA database cases, and forum reports. It described abdominal discomfort, jaundice, itching, and dark urine among common presenting symptoms, while also noting that the evidence was low quality and that the people at heightened risk remain unclear.

A 2021 Drug-Induced Liver Injury Network report described 11 attributed cases: participants were symptomatic, developed jaundice, and most required hospitalization. The report concluded that kratom can cause severe liver injury with jaundice.

These reports cannot give you a personal answer if you already have liver disease. They do make one thing clear: put your existing liver condition on the table before applying broad kratom information to yourself.

What about kidney disease, blood pressure, anemia, itching, or breathlessness?

Kidney disease can be connected with several body-wide problems, but a symptom needs its own medical context. NIDDK lists chronic kidney disease, anemia in chronic kidney disease, high blood pressure and kidney disease, and kidney failure as separate topics.

NIDA reports that case reports involving kratom exposure have included high blood pressure, liver problems, nausea and vomiting, confusion, tremors, seizures, and slow breathing. That does not identify the cause of your high blood pressure, anemia, itching, shortness of breath, or kidney condition. It does mean those details should be discussed together with your medicine list.

Who can review the medicine list with you?

Bring the full list to the qualified professional involved in your liver or kidney care, including prescriptions, nonprescription medicines, vitamins, herbs, drinks, extracts, and any kratom product. Include the product label or a clear photo of it, the form of the product, and the dates symptoms began if symptoms are part of the question.

NIDA says drug interactions may influence kratom’s effects and that case reports involving kratom with other drugs have been associated with severe adverse effects, including liver problems and death. The FDA also warns about serious adverse events including liver toxicity, seizures, and substance use disorder. A complete list helps the reviewer see the whole picture.

Coordinated-care worksheet

Bring or write downWhy it helps the conversation
Kidney or liver diagnosisIt identifies the organ condition that general kratom research may not cover.
Every medicine and supplementNIDA notes that other drugs can influence effects.
Product label and ingredient panelIt distinguishes leaf-based products from extracts or products with additional ingredients.
Batch number and any laboratory documentIt gives the reviewer a specific product record to discuss.
Symptoms and when they beganIt helps place jaundice, itching, dark urine, abdominal discomfort, breathing changes, or other concerns in context.
Questions to askAsk whether your organ condition or medicine list changes the discussion and what follow-up is appropriate.

For help understanding why a batch record and testing document matter, see Lab-Tested Kratom. If your concern is pain after stopping, Pain Care After Stopping Kratom explains that separate topic.

Which symptoms need prompt assessment?

Seek prompt assessment for yellowing of the skin, dark urine, itching, or abdominal discomfort, which the 2020 liver-injury review identified among common presenting signs. NIDA also lists slow breathing, seizures, confusion, high blood pressure, nausea and vomiting, and liver problems among serious effects reported in case reports. If you have an immediate or severe concern, use urgent local medical services.

What the evidence cannot tell you

Research cannot tell you whether kratom is safe for your particular liver disease, kidney disease, kidney failure, anemia related to kidney disease, or high blood pressure. It also cannot predict an interaction from a medicine name alone or turn a product label into a personal safety answer.

As of October 16, 2026, the FDA’s kratom page states that there are no FDA-approved prescription or over-the-counter drug products containing kratom or its known alkaloids legally on the U.S. market, and that the agency warns consumers about serious adverse events including liver toxicity. This is information, not legal advice; state rules may also differ, and the FDA says state health and law-enforcement agencies are the best source for applicable state laws.

What is the appropriate next step?

Use the worksheet above to prepare a short, complete question for a qualified professional: “I have this liver or kidney condition, take these medicines, and am considering or have used this exact product. What should I know about my situation?” That gives you a conversation built around your condition, your medicines, and the specific product—not a broad online answer that may not fit.
